The Bag of Hope Program is part of the Palmetto Chapter’s Outreach Program. The mission of the Outreach Program is to provide education and support to anyone who has a personal connection to diabetes. The Bag of Hope Program connects children and their families with other local families who have a child with diabetes at the time of diagnosis. The bags contain a comprehensive collection of educational and comfort materials intended to introduce a newly diagnosed family to the larger Breakthrough T1D family and its mission to find a cure for diabetes. All Bags of Hope include Rufus the Bear with Diabetes, an Accu-Chek blood glucose monitor, a support dvd, and educational books for children and their caregivers.
If your child was recently diagnosed with type one diabetes and did not receive a Bag of Hope at their diagnoses please download the BOH request form and mail or fax it to the Breakthrough T1D Palmetto Chapter.
If you are an adult that was recently diagnosed with type one diabetes and did not receive an Adult Care Kit at your diagnoses please download the request form and mail or fax it to the Breakthrough T1D Palmetto Chapter.
