T1D Community Connection Program

Are you interested in connecting with individuals/families in a meaningful way to foster relationships by welcoming, supporting, guiding, and connecting them to the larger T1D community and Breakthrough T1D? The Eastern PA Chapter is recruiting to train outreach volunteers to join the T1D Community Connections Program. Each Outreach Volunteer will be assigned a new family and will be asked to reach out to the family once a month for the first year of their diagnosis to engage and support the connection. For example, we would want to invite the new families to the One Walks and to the TypeOneNation Summit. There will be an easy-to-use online tracking tool to help track assignments each month and enter in information about your interactions. This tool also provides you with Breakthrough T1D resources right at your fingertips to easily click and share with families.

Your School is Invited to Join the Breakthrough T1D Kids Walk to Cure Diabetes

The Breakthrough T1D Kids Walk program is an educational, in-school fundraising program with two goals: 1) to educate students about type 1 and type 2 diabetes and the importance of a healthy lifestyle, and 2) to provide them with an opportunity to make a difference by raising money for type 1 diabetes (T1D) research.

Please consider introducing Breakthrough T1D to your child’s school for the 2017-2018 school year to support students and families living with T1D in your community.

When your school joins the growing list of Breakthrough T1D Kids Walk schools around the country, students will raise awareness of the rising incidence of diabetes while raising money for Breakthrough T1D to fund research that will create a world without T1D. Educators and families love this program because it’s easy to implement and has amazing benefits:

  • Students gain basic knowledge about diabetes, healthy eating/exercise, and the importance of community service.
  • Your school can receive 10 percent of the money raised to be used as you choose.
  • Students can earn fun Difference Maker Awards, and your school can earn a recognition banner.
  • Kids Walk gets kids moving! If you already have a walk scheduled at your school, Breakthrough T1D staff can recommend other fun, physical events for your students.
  • Our turnkey program requires minimal time from staff and teachers.
  • It’s all about kids helping kids!

CarlaAnn Henry- Outreach Manager

With over 20 years as a strategic health navigator, CarlaAnn has seen that traditional marketing for resources and programs are often directed towards large demographics. So the population most in need and the population most needed for research studies were not included. Through innovative research, canvassing with advocacy groups, and resource development with community partners, she is proud to have boosted enrollment in research, clinical trials, and solutions. In her earlier career as a mental health technician and case manager, she worked directly with patients, patient groups, hospitals, and providers. As a patient coordinator and advocate, she enjoyed helping patients understand regulations, rights, and research progress that could make a significant difference in their lives.

In her spare time, she is an avid world traveler, loves to spend time at the beach, and enjoys outdoor activities, especially biking and hiking.
