Please share information about yourself\/your family so we can better understand your experience(s) with Breakthrough T1D. You can also provide feedback on where we can improve and what YOU want from Breakthrough T1D.<\/p>\n<p style=\"text-align: center;\"><strong>Take the Mid-Atlantic DE&amp;I Survey: <a href=\"https:\/\/forms.office.com\/r\/rW9S1XHTQT\">English<\/a> \/ <a href=\"https:\/\/forms.office.com\/r\/FFbZb9rPUK\">Spanish<\/a><\/strong><\/p>\n<p style=\"text-align: left;\">Your answers will be anonymous unless you choose to share your name and contact information to have a follow-up conversation.
