Dear Prospective Breakthrough T1D Youth Ambassador,
On behalf of the Breakthrough T1D Desert West Chapter, we would like to invite you to participate in our 2025-26 Youth Ambassador Program.
The Youth Ambassador Program was created to develop youth leaders within the type 1 diabetes (T1D) community. As a Youth Ambassador, you’ll inspire others by sharing your story, raise awareness by educating the broader community about T1D, and directly impact Breakthrough T1D’s mission. You’ll also meet other peers with T1D, enhance speaking and storytelling skills, volunteer at chapter and community events, make new friends and have FUN!
The YA Program is open to children and teens living with type 1 diabetes, between 1st and 12th grade.

EXTENDED DEADLINE: Please submit your completed forms no later than August 25th, 2025.
Once we’ve received all the applications for this year’s program, we will conduct a review and may reach out to you if we have any questions. You’ll be notified by August 28th, 2025, if you’ve been selected for the program.
