Patty Sue Mozart will receive the Impact Award at the 2026 Breakthrough T1D Tee to Table event on Friday, May 8, 2026 at The Ritz-Carlton, Half Moon Bay. Patty Sue’s dedication and contributions to our mission and the type 1 diabetes (T1D) community has been exceptional and exemplary.

Patty Sue has been a dedicated Breakthrough T1D volunteer for nearly 20 years, inspired by her son Forrest’s diagnosis with type 1 diabetes (T1D) at age 15. She previously served on the chapter’s Spring Fling committee, which later evolved into the Tee to Table event, and she went on to chair Tee to Table for six years. Through her leadership, and with the support of the committee, Tee to Table was transformed into the organization’s top golf and gala event, raising over $1.6 million annually for T1D research. She continues to be an active member of the event committee today. In 2024, she also participated in the Breakthrough T1D Amelia Island Ride.

Patty Sue is the proud mother of three adult children: Justin, Ashley, and Forrest, and she is a grandmother to three. We are thrilled to honor her with the 2026 Breakthrough T1D Tee to Table Impact Award.
